SMS Opt-In Evidence

Operated by Phuc Labs Inc · platform: isolace.net · disclosure version: sms_opt_in_2026_09_29_v4

Where a person opts in

A parent or guardian opens the center's published assessment or enrollment link, enters their mobile number, reads the sender-specific disclosure, checks the optional SMS box, and submits the form. The SMS box is not checked by default. Submitting either form without checking it does not create SMS consent. Creating an account alone does not create consent.

Working public example, Kumon of Quincy: assessment form and enrollment form. These two published forms can be opened without signing in. Use these exact public links when reviewing the opt-in experience. The family wizard at /enroll/{center-id}/wizard requires an account and shows the same optional consent in “About you”; the parent checks it and selects Next to save their answer.

These disabled previews use the actual form templates and current disclosure. They do not collect information or subscribe you. Use your own center's link to opt in; the Quincy links above are for Quincy families.

What happens after opt-in

For an assessment request with consent, the confirmation identifies the center. Example: “Kumon of Quincy: Thanks Alex! We received your free-assessment request for Sam and will confirm a time soon. You're subscribed to Kumon of Quincy texts about appointments, attendance and progress. Msg frequency varies. Msg & data rates may apply. Reply HELP for help, STOP to opt out.”

The saved form record includes the consent answer, timestamp, source form, disclosure version and exact disclosure text. Private family records are not published on this page.

Opt-out and help

Reply STOP at any time to unsubscribe; the system confirms the request and sends no further messages. Reply HELP for help, or contact hello@isolace.net. For all supported keyword aliases and their actual replies, see the keyword workflow. The instructions are: Reply STOP to opt out or Reply HELP for help.
